Introduction
Woven Health Clinic, a 501c3 has partnered with the City of Coppell to offer free medical care for Coppell residents until December 31, 2024 or earlier if the grant funds are exhausted. The only program requirement is that the person live in the city of Coppell. Woven Health Clinic is holding pop-up clinics at various locations in Coppell (see flyers) for walk-in appointments and has two permanent Clinic locations and is providing tele-med visits virtually for scheduled appointments. The Coppell Woven Health Program offers free sick visits, well check visits and chronic disease management visits. Patients received free lab work, access to health events, including free cooking classes and exercise classes. Woven Health Clinic provides access to an internal medicine physician, nurse practitioners, registered dietitians, and onsite behavioral health specialists all at no charge for Coppell adult (18 years and older) residents.
Our challenge as a non-profit medical clinic is to get the word out to Coppell residents so that they can access this high quality medical care for free. We would like to have a booth at the Coppell Farmers Market to share this important information with attendees. We will attend as often as you will allow us!
Description of Organization
On Eagles Wings, Inc, Doing Business As Woven Health Clinic is a 501C3, a non-profit community clinic that provides primary health care services and is a medical home for uninsured and under-insured adults that work and live in Carrollton, Farmers Branch, Addison, Coppell and surrounding areas that cannot afford health care. Additionally, we have special programs for Coppell residents, and for small businesses that cannot afford to offer health insurance to their employees. We are a true community clinic.
Our vision is to help people create healthy lives that build healthy communities. Our mission is to provide affordable, quality, accessible healthcare to those that are uninsured or under-insured. Our goal is to provide high quality, comprehensive health care, and empower our patients to PREVENT health problems and MANAGE existing health issues for better outcomes. We focus on delivering high quality, integrated physical and mental healthcare.
Our core values are: patient centric, integrity, compassion, dignity, quality, innovation, responsive and accessibility. Our Board, employees and volunteers live and model these values, especially dignity, quality and innovation.
History
Woven Health Clinic was started thirty-one years ago by Webb Chapel United Methodist Church and Chapel Hill United Methodist Church because they saw a need for medical services for the uninsured in the community. Originally, the Clinic was staffed by 100% volunteers, including administration, physicians, and nurses. Over the last 31 years, it has served over 35,000 patients.
The Clinic has evolved from a volunteer clinic that primarily serves sick people on a random basis to a fully staffed primary clinic that is a recognized National Committee for Quality Assurance (NCQA) Patient Centered Medical Home and provides over 10,000 visits a year.
Major Accomplishments
· Expanded our partnership with UTSW Medical Center to include joint participation in Medicaid Waiver 1115, which is a federal program that services low income, uninsured adults.
· Received National Quality Committee Association (NCQA) Patient Centered Medical Home recognition in December 2017.
· Expanded services from Primary Care only to include onsite specialty services for dermatology, gynecology, and neurology, nutrition therapy and basic mental health care.
· Launched our Chronic Disease Management Program in 2015; expanded it to include Disease Prevention, PREVENT Disease NOW! Program in 2016.
· Partnered with UT Southwestern to implement depression and anxiety screening and treatment. Launched a telemedicine program to provide patients with free mental health counseling and therapy.
· Partnered with Dallas Medical Center and Touch Stone Imaging to provide patients with discounted mammograms, and radiology and digital imaging.
· Partnered with Essilor Eye Vision Foundation to provide an Eye Clinic with free eye exams and glasses for our patients.
· Collaborates with other community organizations including UTSW, Baylor Scott & White, HCA, Dallas Medical Center, Metrocrest Services, PediPlace and Asian Breast Health Network and many others.
· Supported by community partners: Metrocrest Hospital Authority, American Heart Association, The Civic League, HCA, Metrocrest Area Fund, City of Carrollton, City of Farmers Branch, The Town of Addison, The City of Coppell, Dallas Medical Center, Baylor, Scott & White Carrollton Hospital and many churches and other local organizations.
· Expanded our staff to include a Registered Dietitian and Behavioral Specialist to support disease prevention and management programs. Expanded our staff to include a part-time Gynecology Nurse Practitioner and a Family Practice Nurse Practitioner.
· Partnered with City of Carrollton to open a new clinic in the city of Carrollton.
· Partnered with the City of Coppell to offer free medical services to Coppell residents.
· Best In Class for treating Depression – over 50% of patients in remission; Hypertension – 86% of patients controlled; Best in Class for treating Hypertension – 87% of hypertension patients controlled; Best in Class for preventing Diabetes – 13% of prediabetic patients returned to normal.
Programs and Services
The Clinic provides primary care, basic mental health care, access to medical specialists, referrals to social services, access to health and life style screenings, immunizations, cancer and disease screening, medications, lab services, prevention and chronic disease management, patient education, mental/behavioral healthcare, nutrition therapy, and community health and wellness programs.
