These pocket pillow beds are available in multiple sizes. XS - XL. XS comes with pillow insert since it is a custom size. They are designed as pillow shams so they are removable to wash. All other sizes, you are purchasing the outer bedding only, no insert. M-XL are produced as pre-paid custom orders. See sizing chart.
